The CBT for CH Therapy Adherence Protocol (see Appendix 3) enables an assessment to be made of the extent to which the therapist adheres to principles and practice of CBT for CH and offers a useful guide to the steps indicated for each stage of therapy. As such the protocol serves as a useful training aid and fidelity marker if used in conjunction with audio or video taping of therapy sessions. Developing and sharing the formulation The therapist’s next task is to help the client to see that these power beliefs are the client’s own beliefs about the voice.

Exploring beliefs about the power of the voice By trying the various coping strategies described earlier, Tom began to believe that he had more control over the voice. This was used as evidence to support an emerging belief that he had as much power as the voice. Subsequently, challenging the belief that the voice could make Tom harm himself gave further support for the view that the voice is far from powerful. By mid-therapy, Tom reported that the voice had stopped (and it didn’t return for the remainder of therapy).

Joan concluded that there were more advantages in resisting the voices’ commands. Evidence of the likelihood of Joan obeying commands to harm herself or others was explored. It emerged that Joan had never actually complied with direct commands: the only time she had seemed to act in response to the voices was when she had found herself walking in the middle of the road at night. An alternative explanation for this behaviour was proposed: Joan may have been sleepwalking. Any risk of this happening again was resolved by making sure that her external doors were locked at night, so that she could not unwittingly leave the house in the middle of the night.
